Through the Georgia Pediatric Program (GAPP), CrestPoint provides compassionate skilled nursing and personal care services for medically fragile children. For eligible families, GAPP services are covered by Georgia Medicaid.

Our goal is to support each child’s medical needs while helping families feel informed, respected, and supported throughout the care journey.

Who May Qualify?

A child may qualify if they meet program and medical-necessity requirements.

Under age 21
Active Georgia Medicaid
A medically fragile condition requiring skilled care
A physician’s order documenting medical necessity
Able to safely receive care in the home
Common Needs

Children with complex medical needs may qualify.

Eligibility is determined through the GAPP process and may include children with diagnoses or care needs such as:

  • Ventilator dependence
  • Feeding tubes (G-Tube)
  • Seizure disorders
  • Cerebral palsy

Additional Complex Needs

  • Chronic respiratory conditions
  • Neurological disorders

  • Other complex medical conditions

Skilled nursing and personal care centered on the child.

Skilled Nursing (RN & LPN)

  • Medication administration
  • Tracheostomy and ventilator care
  • G-Tube feeding and management
  • Wound care
  • Respiratory treatments
  • Health assessments and vital sign monitoring
  • Care coordination with physicians

Personal Care Services (PCS)

  • Bathing and personal hygiene
  • Dressing and grooming
  • Feeding assistance
  • Mobility and transfers
  • Safety supervision
  • Comfort and companionship
