Family-centered skilled nursing and personal care services for eligible medically fragile children through Georgia Medicaid.
Through the Georgia Pediatric Program (GAPP), CrestPoint provides compassionate skilled nursing and personal care services for medically fragile children. For eligible families, GAPP services are covered by Georgia Medicaid.
Our goal is to support each child’s medical needs while helping families feel informed, respected, and supported throughout the care journey.
A child may qualify if they meet program and medical-necessity requirements.
